Prospective Employee Background Records Required by Act 34 and Act 151/33

Act 34 and Act 151 specify that employees of Pennsylvania public and private schools must undergo background checks. Student teachers and job applicants must follow the procedures listed below in order to secure the required clearances.

  1. Applicant must secure Request for Criminal History Record Information (Act 34) and Child Abuse History Clearance (Act 151/33) forms (available from 228 Chambers Building/UP, school districts, and State Police stations).
  2. Applicant completes Section I on each form.
    • Check box for "Employment/School Employment."
  3. Applicant submits the completed forms along with certified checks or money orders for $10 each to the address on the appropriate form. (Cash or personal checks are not acceptable.)
  4. Background clearance will be returned directly to the applicant within four weeks.
  5. Applicant presents background record to practicum supervisors and prospective employers. (Background records are valid for one year from date of issuance to applicant.)
    In addition to the previously required clearances, students in CI 295, 495B, 495C, 495D/F, 495E and WLED B/C and/or any courses with a field component must obtain the FBI Fingerprint clearance for any field experience after August 1, 2007.
