After consultation with the University Faculty Senate officers in February 1989, the following wording has been recommended for use on the signature page and is to be used for provisional reviews as well as promotion and final tenure reviews. The statement must include the candidate's signature and the date, with the original signature appearing in the original dossier. This will be the last item in the dossier. Do not number this page.

 

 

I have reviewed the contents of my dossier, with the exception of confidential materials, as defined in the HR-23 Guidelines.
