Registration 2012

PLEASE REFER TO "PARTICIPANT QUALIFICATIONS" BEFORE REGISTERING FOR A WORKSHOP!

Fee for entire 4 day workshop: $200.00 (Attendance will not be granted unless payment has been received!)
Fee to attend days 3 and 4 only:  $115.00 (ONLY with approval from PDE) If you want to register for 2 days only please contact Kristy Barton at 814-863-0805 to be instructed how to do so, before filling out this registration form.

 

NOTE:  Materials for the workshop are an additional cost and the responsibility of the registrant.  Ordering instructions will be sent after registration and payment for the workshop is made. Please verify that we have your correct email address below to send this info to.                                                                                        

 

Credit Card payment is strongly encouraged. We are UNABLE to accept credit card payments over the phone.

Please complete the form below, click on "register now" found at the bottom of this page, and then confirm registration on the next page with a credit card payment. If you must pay by check or money order, make it payable to "Penn State University" and mail to:

    * Be sure to include the registrants name on the check.

Cancellation Policy:  

1.   Full refund will be made if cancellation is made at least 2 weeks prior to the start of the workshop.

2.   If cancellation is made less than 2 weeks prior to the start of the workshop, a $75.00 charge will be retained.

3.   A registrant may transfer from one workshop to another ONE TIME ONLY.  If he/she must cancel the 2nd workshop, no money will be refunded and payment will be required for future workshops.

                                    

Please note the following:

  1. Please indicate a second option in the event your first option has been filled.

  2. Payment must be received at least 2 weeks prior to the start of a workshop. Registrants will not be admitted to the workshop if payment has not been made.

  3. All sessions begin at 8:30 AM and end at 4:00 PM

  4. Workshops may be cancelled at the discretion of Penn State University due to weather or insufficient enrollment.

  5. 27.3 contact hours are approved for this workshop by PSNA and NAPNES, based on 100% attendance by each participant.  Excessive tardiness or absenteeism during the workshop could cause your certificate of completion to be withheld and/or make up time required

Important! In order to confirm your registration, you must complete both page one (personal information) and page two (credit card payment). 

If you cannot pay with a credit card, please submit page one and then call (814) 863.0805 immediately to inform us of your method of payment.  No registration will be confirmed until payment is received in our office.  Thank you.
