To Whom It May Concern,

We are pleased to be the recipients of a 3-year, federally funded grant entitled Project Opportunity and Access (POA) to develop information and discussion sessions for faculty and staff about disability legislation and accommodations. Because greater numbers of students with disabilities are attending postsecondary institutions it is imperative that faculty and staff be prepared to work with this constituency in order to make their higher education experience a positive one. We invite you to participate in this exciting project by completing the Faculty and Staff Survey of Disability Knowledge.

Your participation in POA will greatly improve the quality of services provided to students with disabilities, as well has enhance your own knowledge and ability to meet the needs of the students with whom you will come in contact. The following survey should take no more than 15 minutes to complete. Once you submit your answers, you have the option of obtaining the correct answers and/or linking to other web sites which will provide you with additional information.

Your participation in POA is voluntary. Any responses will be kept confidential; your name will not be associated with your survey responses in any way. You are free to discontinue your participation at any time, or to decline to answer any specific questions. If you accept our invitation to participate in POA, please complete the survey. Your completion of the survey will constitute your consent to participate in this worthwhile project. The survey results have been and are being used to determine the content of various training and information sessions for faculty and staff.  Any questions or additional information needed regarding this project should be directed to Sharon Salter, (814) 863-2421 or email sfs8@psu.edu, or Dr. Jim Herbert, (814) 863-3421 or email jth4@psu.edu.

Sincerely,

Sharon F. Salter, M.A.E.
